Em inglês não tem gêneros, pode ter sido não um cirurgião, mas sim UMA cirurgiã.

Tanto poderia ser UMA cirurgiã, como poderia ser um casal gay que adotou o filho, como o cirurgião poderia estar se referindo ao pai do machucado, quando entram no hospital!
a charada está em inglês, vou comentar em inglês mesmo... The first explanation makes no sense... how can the surgeon be a woman? That's preposterous... to be a surgeon, you need to have BALLS. The second possiblity, that they are a gay couple with an adopted son, is more reasonable, but only if they are from San Francisco.
